Personal Details Collection Statement & Privacy Statement 

Jedrek Hagedorn Clinical Psychologist is committed to protecting the confidentiality of you and/or your child’s medical records, in accordance with applicable Privacy law requirements.

In order to provide their services, Jedrek Hagedorn Clinical Psychologist requires the collection of personal information relating to you and/or your child (as applicable), and will ensure that you and/or your child’s confidential information is respected and kept secure (electronically and physically).

Personal information and health information may be passed onto other health providers including but not limited to other medical professionals, pathology services, radiology services, and legal services (as applicable) in order to provide health care for you and/or your child. It may also be passed onto the Australian Government Department of Human Services for billing purposes.

Your personal information may be disclosed in order to comply with a legal requirement, or when it is necessary to lessen or prevent a serious threat to a patient’s life, health or safety, or public health or safety.

Consent

By using Jedrek Hagedorn Clinical Psychologist's services, I consent that the information collected by this practice may be disclosed to other health care providers and for billing purposes, and I give authorisation for this on behalf of myself and/or my child (as applicable).

I understand this personal information is protected by law, including the Privacy Act 1988, and is collected by the Australian Government Department of Human Services for the assessment and administration of payments and services.

This information is required to process your application or claim.

Your information may be used by the department or given to other parties where you have agreed to that, or where it is required or authorised by law.

I agree to the assignment of the Medicare Benefit (where applicable) being paid directly to Mr Jedrek Hagedorn.

This includes bulk billing benefits for Telehealth Services.

Fees

Using a Mental Health Care Plan provided by a GP, the Medicare rebate for a 50 minute consultation with a Clinical Psychologist $129.50.

Jedrek charges an out of pocket cost according a sliding scale model which considers the patient's financial circumstances. Out of pocket costs vary from $70 to $100.

For those working full time an upfront fee of $230 is payable, while for those working part time a fee of $200 is applicable.

All fees are payable on the day of the appointment, either via EFTPOS or electronic funds transfer. 

If the client wishes for Jedrek to process their Medicare rebate on their behalf, they will need to send a copy of their mental health care plan to him via e-mail no less than a week prior to the session.

Otherwise the client is able to contact Medicare themselves to obtain their rebate directly. 

Cancellation Policy

In order to cancel or reschedule a session with Jedrek, a minimum of 72 hours notice is required, in order for Jedrek to be able to offer your session to another patient.

If you cancel or reschedule and provide less than 48 hours notice, a cancellation fee of the full fee of the session will be applicable.
